For urgent social work assistance after hours and on weekends, page the on-call social worker at 415-443-6778.
In the event the on-call SW does not respond to a page, you can call SW director Rebecca Gates at 415-404-5267. This number is for BCH Supervisor use only and not to be shared.
Social Work supplies, including taxi vouchers, Lyft codes, meal coupons, and other documents are kept in a binder in the Supervisor office.

SCAN Team
SCAN team is available 24/7 in Voalte. The team is helpful in dealing with situations to understand risk and ethics perspectives with CPS considerations. Can often contact CPS supervisors directly. Under SCAN in Voalte.

Families must live greater than fifty miles from BCH SF to be eligible for RMH.
Hotels
BCH Supervisors can book hotels, paid for by UCSF, for urgent housing needs after hours. Use this list to call hotels to determine availibility. Once a hotel is selected, fax or email the information at the bottom of the booking list to the hotel (email only for the hotels without a fax listed). Fax the request form to Kelly Smith, SW office manager, at 415-353-2190. Place a copy of the hotel request form in the sleeve in the nursing supervisor binder, and SW will collect for their records.

Parking
One parking discount ticket will be needed for EACH day of parking. Greater than 3 hours will be charged for a FULL day. If someone has parked for 1 day and 3 hours they will need 2 tickets.
Do not Date Tickets
Parking cashiers will date the tickets when customers are exiting the garage. Cashiers will return a portion of the ticket to the customers for IN and OUT privileges on the current date. IN and OUT privileges are valid until Midnight and then a new day starts. Log the parking sticker into log book.
Taxi

Write family name on the numbered lines, left side. Trip begins at1975 4th Street, write destination address on right side lined area. The patient/family will call the Taxi. Give white copy to patient and pink copy in SW binder.

3). You will receive an email from Lyft
4). Click on the link below to open desktop application
5). Click on “Continue as you”
6). Enter First and Last name
7).Enter cell phone number of rider
8) Enter pick up address (1975 4th St if leaving from BCH)
9). Enter patient's destination address. (Check city)
10). Can leave a note to the driver
11). Lyft will send a text to the RIDER with information, the screen will have the driver and car description.
