OB GYN Patients on BCH Acute and Transitional Care

Guideline for Admitting OB-GYN Patients to BCH Acute and Transitional Care

  • Regardless of age, a pregnant patient being admitted for an issue related to the pregnancy should be admitted by the OB-GYN team to the Birth Center.
  • Other admission situations are summarized below:

 

Not pregnant, admitted for a primary GYN issue

Pregnant, admitted for a medical issue not related to pregnancy

< 18 years old

≥ 18 years old

< 18 years old AND < 20 weeks gestation

< 18 years old AND > 20 weeks gestation

 

OR

 

≥ 18 years old at ANY gestational age

  • Admit to BCH, on a pediatric   primary team

 

  • Specific unit based on diagnosis/acuity

 

  • Consult GYN

 

  • Consider: consult  Adolescent Med (especially for issues related to contraception)
  • Admit to Adult Med/Surg (Adult Hospital) on OB-GYN primary team1
  • Admit to BCH, on a pediatric  primary team

 

  • Specific unit based on diagnosis/acuity

 

  • Consult GYN

 

  • Consider: consult  Adolescent Med
  • Admit to Birth Center (Adult Hospital) on OB-GYN primary team 2

 

  • Consider: consult Adolescent Medicine, Pediatric Hospital Medicine, or other relevant pediatric specialists as needed

This is a guideline only. Specific cases with unique circumstances require flexibility and case-by-case discussion.

 

1  Sometimes, a patient ≥ 18 yo with history of being followed by pediatric specialists for a chronic childhood illness might best be served in the Children’s Hospital (until more officially transitioned to adult care).

 

2 If children (pregnant or not) are admitted to the Birth Center, BCH support resources are available as needed (e.g. Nursing resources, Social Work, Child Life, Pharmacy, Pediatric Hospital Medicine, Adolescent Medicine, or Pediatric Specialty consults, as needed).
